We do not exclude people, treat them differently, or deny access to home health care services based on race, color, national origin, age, disability, sex (including pregnancy, sexual orientation, and gender identity), religion, or diagnosis.

Auxiliary Aids & Language Assistance Services

To ensure effective communication and equal access to our care, A-Z Home Care Options provides the following services free of charge and in a timely manner:

  • Services for Individuals with Disabilities:
    • Qualified sign language interpreters.
    • Written information in accessible alternative formats (such as large print, audio, or accessible electronic formats).
  • Language Services for Individuals whose Primary Language is Not English:
    • Qualified telephonic or in-person interpreters.
    • Essential agency documentation and care information translated into other languages.

If you need auxiliary aids, reasonable modifications, or language assistance services, please speak with your provider.

How to File a Grievance

If you believe that A-Z Home Care Options has failed to provide these services or discriminated against you in another way, you may report your concern to our Compliance Department via phone, online or by mail to the address below:
